All members must have an updated Enrollment Form - Construction on file with the Family Medical Care Plan.  Once enrolled, visit the participant portal to check on claims and manage your special funds account.  To register and to access the participant portal, click here.

Make sure to include a marriage license for spouse coverage and birth certificates for any dependent children being covered.
